• Copy of Birth Certificates for Cheer and for Football for players registering for the first time

• Check for the fee and check for $250 volunteer buy-out option if not volunteering

Please bring fill out these forms and bring them with you on the equipment issue day.

• Physician's Report

• Work Bond deposit check for $250 if you don’t choose the Volunteer buy out option.

• Check for $250 (Football) & $150 (Cheer) for deposit (refundable when the equipment is returned)
